Reimbursement Codes

You can use these codes with your insurance company to see if you can get a reimbursement for your sessions and services.

• Initial Consultation, 60-minutes (Evaluation) — CPT code 90791
• Individual Psychotherapy, 60-minutes follow-up — CPT code 90837
• Individual Psychotherapy, 45-minutes follow-up — CPT code 90834
• Family/Couples Psychotherapy, 45-minutes follow-up — CPT code 90847
• Family/Couples (w/o patient present) Psychotherapy, 45-minutes follow-up — CPT code 90846

Insurance

Depending on your current health insurance provider or employee benefit plan, it is possible for services to be covered in full or in part. Please contact your provider to verify how your plan compensates you for psychotherapy services.

I’d recommend asking these questions to your insurance provider to help determine your benefits:

• Does my health insurance plan include mental health benefits?
• Do I have a deductible? If so, what is it and have I met it yet?
• Does my plan limit how many sessions per calendar year I can have? If so, what is the limit?
• Do I need written approval from my primary care physician in order for services to be covered?

Payment

I only accept debit and major credit cards. Unfortunately cash, check, and or money orders are NOT accepted.

Cancellation Policy

If you do not show up for your scheduled therapy appointment and you have not notified us at least 24 hours in advance, you will be required to pay a $100 late cancellation or no-show fee. Recoupment of fees will automatically occur with the card on file. Individuals are ineligible to schedule a new appointment with an outstanding balance. Once the balance is paid in full, therapeutic services may resume.
